Home and Community Support
At Sharing Moments we recognised that everyone has unique needs and goals, and that each participant requires their own tailored support schedule.
We are dedicated to delivering high quality community and NDIS in-home support that will help participants make the most of their funding and reach their goals. Providing a range of home support services that vary from in-home assistance with household tasks like cleaning and gardening, right through to community support services and life skills development. We take a tailored approach to match support workers with participants to support you in achieving increased independence and goals.
Some of the tasks that we can support you with;
- Household Tasks – Cleaning and tidying around the home and garden maintenance.
- Community Support – One-on-one social outings or help to connect with your community.
- Life skills development – We support the development of skills to promote your independence.
- Meal Preparation – Assistance to prepare meals on your behalf or to assist you.
- Shopping – Assistance at the shops or we can complete the shop for you.
- In home respite – Support to help loved ones take a break when needed.
- Transport – Transport to and from medical appointments and social outings.
